This form is used by the person who issued the subpoena to seek permission to inspect a subpoena.
By completing the form the person who issued the subpoena must confirm the subpoena has been served on each other party and any interested person in the proceeding including the Independent Children’s Lawyer if one is appointed.

Filing with the Court
This form cannot be filed until on or after the day of production.
