When is this form used?

This form is used when a person wants the Court to review an administrative decision or conduct under the Administrative Decisions (Judicial Review) Act 1977.

What else is filed with this form?

An affidavit - general federal law or statement of claim or points of claim.

You also must provide the Court with a copy of the decision and any statement of reasons for the decision.
