This form is to be used in a Priority Property Pool Case (PPP Case) once the Court makes a declaration or notation that the case is designated as a PPP Case.

You must file this form if you:
- file an Initiating Application or Response to Initiating Application seeking the division of a property pool valued at $550,000 net or less (excluding superannuation), or
- you are directed by the Court to file this form.
