This form is used to make an application for final orders pursuant to the Bankruptcy Act 1966 or Bankruptcy Regulations 1996 in a bankruptcy proceeding. This form cannot include an application for interlocutory orders.
An Affidavit is not required to be filed with a Bankruptcy – Application.
